2022 SAMC Apparel Order Form

Club Apparel

This year we are offering SAMC branded face masks, ball caps, beanies, jackets, polo shirts and t-shirts with embroidery available for the polo’s and jackets.

Polo’s and T-Shirts are available in both men’s and women’s sizes, women’s are V-neck. Jackets are uni-sex. T-Shirts’s are printed on both sides. Polo’s embroidered on the front only; Jackets are embroidered on the front, and printed on the back. Embroidered names may be added to both the polo’s and jackets for an additional $5.

We've provided this form for members to use to pre-order club merchandise. The form will total your choices and you may pre-pay your order using PayPal. If you don't complete the PayPal purchase you can pay for your order when you pick up. We will keep the form open through April 30th. After 4/30 we will place the orders received, once they come in member can pick up their orders .
